Appointment Policies (Please read)
- Clicking submit will result in a confirmed appointment. The time slot will be reserved for you. Please only schedule appointments you intend to keep.
Exception : For appointments scheduled within 24 hours, please only come in after we confirm with you. If you haven’t heard from us please call 972-597-2227. - Please ensure accurate contact information is entered. We may cancel the appointment if we are unable to reach you.
- No shows and cancellations/reschedules without 24 hour notice will result in a $35 charge. (24 hours is defined as 1 business day).
- If you are covered by insurance, you must call us at 972-597-2227 to provide your insurance information at least 48 hours prior to your appointment. If we do not receive this information by then, we will assume the appointment is abandoned and cancel it.
- For HMO insurance, please ensure that we are selected as your primary office. Failure to do so may result in a delay in us seeing you.
- New patients with braces will be required to have their wires removed prior to their new patient exam and cleaning. This is essential for us to ensure we can provide the best possible care.
